SEND & Inclusion 

At Brookfield School, we are committed to providing an inclusive, nurturing and aspirational environment where every pupil can thrive. All our pupils have an Education, Health and Care Plan (EHCP) for Social, Emotional and Mental Health (SEMH) needs alongside associated Special Educational Needs (SEN). We focus on understanding each child as an individual and delivering provision that supports their progress, wellbeing and future opportunities. 

 

Our Approach to SEND 

Our provision-led model ensures that every pupil receives a needs-led, carefully planned package of support. Each provision is led by a Provision Leader who oversees day-to-day provision and acts as the main point of contact for pupils, parents and professionals. They provide a consistent link for any matters relating to EHCPs, support plans or the annual review process. 

We are committed to delivering personalised learning within a safe, supportive and predictable environment. Our approach aims to reduce barriers to engagement, promote independence and resilience, support emotional development and ensure every pupil accesses a broad, balanced and meaningful curriculum. 

 

Leadership of SEND & Inclusion 

SEND across the school is overseen by the Headteacher and Fiona Carver (SENCO / Deputy Headteacher), who provides strategic leadership, professional guidance and ongoing support to Provision Leads. She is supported by two additional trained SENCOs, ensuring depth of expertise and continuity across all Brookfield sites.

Curriculum, Support and Interventions 

Our curriculum is designed to be adaptable, accessible and responsive to the needs of our pupils. Support may include targeted academic intervention, therapeutic approaches, personalised strategies for emotional regulation, and opportunities for small-group or one-to-one work. We use positive, relational approaches to behaviour and place equal value on academic learning, personal development and wellbeing. 

Enrichment activities and wider experiences are embedded throughout the year to help build confidence, develop social skills and prepare pupils for future pathways. 

 

Working in Partnership 

Strong relationships with families are at the centre of our SEND provision. We work closely with parents and carers, ensuring they are involved in planning, reviewing and celebrating progress. Communication is regular, open and supportive, helping families feel informed, included and confident in the support their child receives. 

We also have access to a range of external professionals, such as Educational Psychologists, Speech and Language Therapists, CAMHS, Occupational Therapists and Social Care, enabling pupils to benefit from coordinated, multi-agency support. 

 

EHCPs and Annual Reviews 

EHCPs underpin all planning at Brookfield School. The outcomes in each plan help shape provision, identify priorities and guide our assessment of progress. The annual review process is collaborative and forward-focused, involving parents, pupils and relevant external agencies to ensure the plan continues to reflect each child’s developing needs. 

 

Consultations and Admissions 

New consultations and placement requests must be made through Lancashire County Council’s Inclusion Service, which oversees the placement of pupils with SEND across the county.
